For your convenience we accept Visa, MasterCard and Discover. We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered.
Co-pays, deductibles, and amounts over the usual and customary charge will be expected at your appointment. We will complete necessary insurance forms as a service to you. Please keep in mind dental insurance is a contract between the insured (patient) and the insurance company. The patient is responsible for any balance due after the claim has been paid by your insurance company. Any balance remaining after 30 days is subject to a non-refundable 1.5% monthly finance charge (18% annually).
Please understand if payment is not received in a timely manner from your insurance carrier or any unpaid balance exists after the insurance payment, you will be responsible for the balance.
Patients with Delta Dental Insurance:
Only select Delta Dental plans will pay our office directly. Delta Dental is the only insurance company that chooses to operate in this manner. Unfortunately, it penalizes the patient in an attempt to limit your opportunity to seek treatment outside of their network. For the plans that do not pay our office, the full payment is due at the time of treatment. Payment is also due in full for all patients covered with the State of Illinois insurance administered by Delta. As a courtesy, we will complete and submit the claim to your insurance company.
